Record decisions

Create a decision when the team chooses between meaningful alternatives or establishes a constraint that future work must respect.

Include:

• the context and decision pressure;
• the chosen direction without implying unapproved product or architecture choices;
• alternatives considered;
• consequences and follow-up work;
• links to affected scope and architecture records.

Keep provisional discussion out of durable decision language until it has been reviewed.
