Maintain documentation data

Scope records live under data/scope/ with one record per .yml file and one deterministic index.yml per collection.

Kanban state lives in data/kanban/board.yml. Empty arrays are valid and render explicit empty states. Do not infer cards or focus items from commits, issues, or conversation.
